The Lebanese American University Medical Center - Rizk Hospital is seeking a closed Bid for CCTV surveillance cameras and video System as per attached specifications in tenders RM009-2017 and RM010-2017. The supplier should send 2 hard and soft files : 1- The Technical and Business Proposal must include the following: • Brand and Origin of each component of the Solution • Reference of each component of the Solution • Technical requirements for installation and implementing the Solution • Brochures for each offered component of the Solution • Professional Services required for the implementation of the Solution • Project Implementation plan of the Solution • Company profile 2- The Financial Proposal must include the following: • Price per item delivered to LAUMC-RH (excluding VAT) • Warranty period • Maintenance and Support Fees (clearly stated) after the warranty expiry • Offers (discount , free goods or other means) must be reflected clearly and separately • Price list of disposables and spare parts. These files should be delivered to the procurement Manager as per the contact and address below no later than 12:00PM on 28/06/2017. The response should consist of the completed forms that are provided as part of this document as well as any other information to support the supplier’s response. The submitted documents and soft copy should be included in a sealed envelope stamped with the company name and the bid number must be clearly shown. The bid should be presented in 2 original copies: The Bids should include the following: o Brand and Origin of each item. Reference of each item. o Catalogues & International Licenses or Certificates of the Items in question. o Brochures and Samples (When possible) of each offered item. o Prices per item delivered to LAUMC-RH (excluding VAT). o Offers (discount, free goods or other means) can be reflected separately. o Shelve life (expiry date) of the items (when applicable) o A Compliance sheet should be filled and send as soft and hard copies stamped and signed by company. o Release date of offered model. o Delivery lead time o Payment terms. o Short term &Long term spare parts with their respective discounted prices and life spam for long term spares. o Availability of spare parts and accessories (when applicable) for a minimum period of 10 years. o Any related disposable items with corresponding discounted price list o Minimum 2 years warranty including labor & parts o Maintenance fee after warranty period o Free training for concerned parties o List of References and Models sold in Lebanon All proposals inquiries should be addressed to: Eleonore Maamary Procurement Manager Telephone: 01-200800 Extension 5026-5528 Email: eleonore.maamary@laumcrh.com Thank you
