The Clinical/Staff pharmacist is responsible for efficient, secure and proper pharmacy operations, provides comprehensive medication management services to patients that integrate clinical services, order management, patient/staff education and research.
DUTIES & RESPONSIBILITIES (including but not limited to)
 Ensure accuracy, conformity and timeliness of pharmaceutical preparations.  Verify medication orders, clarifying any questionable, inaccurate or incomplete orders as necessary.  Collaborate effectively with physicians and nurses to facilitate patient outcomes based on safe Pharmacy practice.  Apply clinical and distribution skills necessary to provide safe, efficient and effective pharmaceutical care to our patients.  Compound chemotherapy preparations.  Ensure pharmaceutical products are stored appropriately and securely to maintain potency.  Track expired products.  Prepare purchase requests for the pharmacy.  Receive and check the pharmaceutical products  Participate in the inventory process.  Actively participate in quality management including documentation of interventions, policies, and accreditations.  Utilize all reference material needed to provide pharmaceutical care.  Stay abreast of new or reformulated products on the market.  Supervise and coach the work of technician aids and supply clerk in the preparation and dispensing of all pharmaceuticals before they leave the department.  Lead process improvement activities involving medication distribution. Clinical Responsibilities and Duties:  Round as a member of the interdisciplinary teams as needed for coverage.  Verify the accuracy and process medication orders.  Prevent, detect and resolve medication related problems and provide evidence based patient specific drug information and disease state management.  Participate in clinical guideline development, implementation and assessment, disease management and drug use programs as identified and in coordination with the Clinical Coordinator.  Identify process improvement initiative to avoid preventable errors and improve patient safety related outcomes.  Demonstrate teamwork with the Pharmacy to ensure that medication is communicated and delivered timely, safely and effectively.  Serve on hospital committees pertaining to Medication Safety  Collaborate on the achievement of departmental goals and objectives.  Participate in teaching of students and residents.  Attend/participate in pharmacy conferences and grand rounds.  Respect and adhere to the mission, vision, and values of LAUMC-RH.  Provide staffing support and on call duties to the inpatient pharmacy on an as needed basis.  Follow and ensure strict safety procedures.  Initiate actions to predict the occurrence of nonconformities on equipment and services.  Establish and monitor corrective and preventive actions  Perform other duties pertinent to this job as appropriate or as required.
 Pharm D with a minimum of two years of hospital pharmacy experience, with extensive exposure to direct patient care, pharmacy operations and automation.  Completed a Pharmacy Practice Residency (PGY-1) and or board certification is preferred.  Licensed as a pharmacist in Lebanon.
 Maintain competency in unique aspects of drug therapy.  Display evidence of leadership in pharmacy practice and professional associations.  Display evidence of expertise in education/training of students and residents.  Computer competency especially in Microsoft Office (Word, Excel, Outlook, and PowerPoint).  Excellent oral and written English communication skills. French is a plus.  Able to work within a professional code of ethics and respect confidentiality.  Ability to work with others as part of a team.  Time management and organizational skills.  Reliable and dedicated.  Able to be tactful in hectic and stressful situations.
